APPLICATION FORM TO THE DATA CONTROLLER UNDER THE LAW ON THE PROTECTION OF PERSONAL DATA

1. General Information

Article 11 of the Law No. 6698 on the Protection of Personal Data ("KVKK") grants data subjects (Applicants), defined as relevant individuals, the right to make certain requests regarding the processing of their personal data.

In accordance with Article 13, paragraph 1 of the KVKK (Law on the Protection of Personal Data), applications regarding these rights must be submitted to our company, which is the data controller, in writing (with a wet signature) or through other methods determined by the Personal Data Protection Board ("Board") (via the applicant's registered electronic mail address "KEP", secure electronic signature, mobile signature, using the electronic mail address previously notified to the data controller by the data subject and registered in the data controller's system, or through a software or application developed for the purpose of the application).

In this regard, for written applications to our company, a printout of this form should be submitted;

• The applicant must submit their application in person, in writing, and with a wet signature.

or

• Applications can be submitted to us through a notary public.

• The applicant may submit this form, or a form containing the required information requested on this form, in digital format.

3. By signing with a mobile signature.
4. via the email address he/she previously provided to us and which is registered in our system.
5. Through the application form found on the website of ETY Yapı Sanayi Anonim Şirketi.

The table below provides information on the specific written application channels through which applications can be submitted to us.

Furthermore, once other methods determined by the Board are announced, our company will also announce how applications will be received through these methods.

In accordance with Article 13, paragraph 2 of the Personal Data Protection Law (KVKK), your applications will be answered within 30 days of receiving your request, depending on its nature. Our responses will be delivered to you in writing or electronically, as per Article 13 of the KVKK.

This application form has been prepared to determine your relationship with our company, to fully identify any personal data processed by our company, and to ensure that your application is answered accurately and within the legal timeframe. To eliminate legal risks that may arise from unlawful and unfair data sharing, and especially to ensure the security of your personal data, our company reserves the right to request additional documents and information (such as a copy of your national identity card or driver's license) for identity and authorization verification. Our company does not accept responsibility for any claims arising from inaccurate or outdated information or unauthorized applications submitted through this form. If the processing of your request requires additional costs, a fee may be charged as stated in the "Notification on the Procedures and Principles for Applications to the Data Controller". A processing fee of 1 Turkish Lira may be charged for each page exceeding ten pages. If the response to your application needs to be provided on a recording medium such as a CD or flash drive, you may be charged a fee equivalent to the cost of that recording medium.
