APPLICATION FORM TO THE DATA CONTROLLER UNDER THE LAW ON THE PROTECTION OF PERSONAL DATA<\/strong><\/p>\n

  1. General Information<\/strong><\/li>\n<\/ol>\n

    Article 11 of the Law No. 6698 on the Protection of Personal Data (\"KVKK\") grants data subjects (Applicants), defined as relevant individuals, the right to make certain requests regarding the processing of their personal data. <\/p>\n

    In accordance with Article 13, paragraph 1 of the KVKK (Law on the Protection of Personal Data), applications regarding these rights must be submitted to our company, which is the data controller, in writing (with a wet signature) or through other methods determined by the Personal Data Protection Board (\"Board\") (via the applicant's registered electronic mail address \"KEP\", secure electronic signature, mobile signature, using the electronic mail address previously notified to the data controller by the data subject and registered in the data controller's system, or through a software or application developed for the purpose of the application). <\/p>\n

    In this regard, for written applications to our company, a printout of this form should be submitted;<\/p>\n

    • The applicant must submit their application in person, in writing, and with a wet signature.<\/li>\n<\/ul>\n

      or<\/p>\n

      • Applications can be submitted to us through a notary public.<\/li>\n<\/ul>\n
          \n
        • The applicant may submit this form, or a form containing the required information requested on this form, in digital format.<\/li>\n<\/ul>\n

          1. via his\/her registered electronic mail (KEP) address<\/li>\n
          2. By signing with a secure electronic signature.<\/li>\n
          3. By signing with a mobile signature.<\/li>\n
          4. via the email address he\/she previously provided to us and which is registered in our system.<\/li>\n
          5. Through the application form found on the website of ETY Yap\u0131 Sanayi Anonim \u015eirketi.<\/li>\n<\/ol>\n
